Birth Records

The Columbiana County Health District has the ability to issue certified birth certificates for any individual born in the State of Ohio from 1908 to present, regardless of what county you were born in.

The cost is $27.00 per certified copy. Our office is open Monday-Friday 8am-4pm. Closed all legal holidays.

Vital Statistics offers the following services:

  • Walk-in: Certified copies of birth certificates can be obtained by completing a short questionnaire. No credit/debit cards or out-of-state checks accepted.
  • Order On-Line at: Columbiana County Health Department (OH) | Order Certificates - VitalChek  Cost per certificate is $27.00 plus a $7 VitalChek processing fee. Overnight shipping is available for an additional $19.00. VitalChek does accept credit and debit cards for on-line orders.
  • Mail-In:  Mail completed application to our office along with a cashier’s check or money order in the amount of $27.00 payable to Columbiana County Health District. Our office does not accept credit/debit cards or out-of-state checks. Once your request is received, the certificate will be mailed same day, when possible. Refunds will be issued only for orders where a document cannot be found.
  • Genealogy information: Birth and death information from 1908 to present is available at the Health District’s office or by calling 330-424-0272, extension 115. Any request for genealogy will be conducted when time permits and is not given priority status.

Death Records

This office only issues certificates for deaths that occurred in Columbiana County, Ohio (outside city limits of East Liverpool and Salem) from December 1908 – present.

Records prior to 1908 can be obtained by contacting Columbiana County Probate Court, by calling the Ohio History Connection at 1-800-686-6124, or by contacting the Ohio Department of Health.

For deaths occurring outside Columbiana County but within Ohio, contact the county office of vital statistics where the death occurred.

Social Security Numbers on Death Certificates

Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent's death, a decedent's social security number shall not be included on a certified copy of the decedent's death certificate unless that information is specifically requested to be on the certified copy by one of the following who presents proof satisfactory to the director, state registrar, or local registrar of the person's identity:

Those able to receive a death certificate with the decedent’s social security number include: 

  • The decedent's spouse;
  • A county veterans service officer employed under section 07of the Revised Code
  • An official specified in division (A), (B), or (C) of section 15of the Revised Code who is authorized to cause the burial or cremation of a dead person as described in that section
  • A lineal descendant of the decedent
  • A private investigator
  • An official of the federal or state government or of a political subdivision of the state charged by law with detecting or prosecuting crime
  • A member of the media with proper accreditation
  • The executor or administrator of the decedent's estate
  • A licensed funeral director, or an employee or agent of that individual, who requests a certified copy of the decedent's death certificate on behalf of a person described in division (A)(5) of this section other than a person described in division (A)(5)(g) of this section;
  • Any person who is authorized by law to act on behalf of the decedent or the decedent's estate

Proof of satisfactory identification may include, but not limited to, a state issued ID.

 


NOTE: Pursuant to ORC 3705.29 (A) (3) No person shall purposely obtain, possess, use, sell, furnish, or attempt to obtain, possess, use, sell or furnish to another for the purpose of deception any certificate, record, or report required by this chapter or certified copy of it that relates to the birth of another person, whether living or dead.
